Frequently Asked Questions

Whether it’s your first time in therapy or you’re searching for a new therapist in this stage of life, we encourage you to ask any questions you may have. Browse through some common questions or reach out, if it’s not covered here.

  • We provide individual therapy services via telehealth. We offer a complimentary 15-minute consultation to discuss what is bringing you to therapy at this time, what you are looking for from a therapy provider, and the type of support we can provide. At that time, we can answer any questions you may have about the therapeutic process and if we decide to move forward we will work together to schedule a session.

Sessions are typically held once per week. In some cases, there may be times when more than one session a week is necessary. The therapist will assess each individual client uniquely and therefore session frequency is a conversation you can have in your work with your provider.

  • Yes, we are in-network with BCBSIL PPO, Aetna, and BCBS HMO Site 447, 243, and 312 (NorthShore Physician Associates). If you are not in-network with any of these insurances and are interested in paying out of pocket, we can provide a monthly “superbill” outlining services rendered, which you can submit directly to your insurance company for possible reimbursement.

If using your insurance, please contact them prior to starting services to inquire about your behavioral health coverage.

*Please note: there are times when insurance representatives misquote benefits. If your insurance does not pay for services, you are responsible for the full contracted rate. You, not your insurance company, are responsible for the payment of services.

If you are interested in paying out of pocket for therapy services our rate is $175 for the intake session and $160 for all subsequent sessions.

  • We ask that if you need to cancel or reschedule an appointment please notify your therapist at least 24 hours in advance of the scheduled appointment. If a 24-hour notice is not given, you will be responsible for the full payment of the missed session. We charge $100 per session if missed or canceled within 24 hrs. Please note that insurance will not cover the payment for a missed appointment.

  • Crescent Moon is an all-telehealth practice located in Chicago, IL. We can see clients who reside anywhere in Illinois. We see clients virtually through a HIPAA compliant video platform.

  • By law, according to the No Surprises Act H.R. 133 which went into effect on January 1, 2022, health care providers need to give clients or patients who do not have insurance or who are not using insurance an estimate of the bill for treatment services. This applies to patient who are paying out of pocket for services.

    * This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created.

    * You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes (under the law/when applicable) related costs like medical tests, prescription drugs, equipment, and hospital fees.

    * The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill.

    * If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
